Attendant Care or ACT 150 Waiver
ACT 150 provides supports and services so people with physical disabilities can continue to live at home. The state-funded services are offered for a fee to persons who do not qualify financially for Medicaid. Eligibility is based on a level of care assessment, a financial eligibility determination and program eligibility.
To Be Eligible for Act 150 Program Services, You Must:
- Be a resident of Pennsylvania.
- Meet the level of care needs for a Skilled Nursing Facility.
- Be between 18-59 years of age.
- Be capable of
- Hiring, firing, and supervising attendant care worker(s)
- Managing your own financial affairs
- Managing your own legal affairs - Be capable of directing your own care.
- Have a medically determinable physical impairment that is expected to last of a continuous period of not less than 12 calendar months or that may result in death.
- Proceed through the eligibility process for the Medicaid (MA) program and be determined financially ineligible.
- Require assistance because of physical impairment to complete daily living functions, self-care and mobility, including but not limited to, those functions in the definition of Personal Assistance Services.
- May have a low co-payment based on your income. The co-payment will not be more than the total cost of services.
